• AMA Conference Member Fee: $20 per person (regularly $30)

1. You can purchase your conference registration by CHECK or CREDIT CARD. .
2. Whether you are paying by check or credit card, complete this online survey. A copy of this survey must be completed for each person attending the conference.
3. Please send your payment within one week of completing this online registry. Registrations are due by Wednesday, September 30th, 2009!
4. Payment must be received prior to conference date to guarantee registration!
